NAME CHANGE

PACKET INSTRUCTIONS

Below are the Self-Help Center's name change form packets. It is your duty to choose the correct packet.  The Self-Help Center staff is prohibited from instructing customers on which packet to use. 

To choose a packet please read the brief description of each packet.  If you are still unsure of which packet to use, open each packet and read at least the first two pages of the instructions. You may use a packet if all of the statements under the heading "Explanation" apply to your particular situation.  If all of the statements do not apply to your particular situation then that packet is not the correct packet.

All of the packets are in a PDF (Portable Document Format).  In order to view and use the packets you must have Adobe Acrobat Reader 5.1 or higher.  If you do not, you can download it for free by clicking here.

Please be sure to print all pages because missing pages may render your document unacceptable to the courts.

NAME CHANGE FORM PACKETS

Petition for Change of Name for an Adult – If you are 18 or over and wish to legally change your name, complete the following documents.  You must publish notice of your proposed name change in a newspaper for three weeks.  You may submit your request for a final order changing your name 10 days after the publication is complete.

Petition for Change of Name for a Minor Child – If you would like to legally change the name of a child, complete the following documents.  Please note that typically, BOTH PARENTS must consent to the name change.  If you cannot obtain the other parent’s consent, contact the judicial law clerk (after you know which judge is assigned to your case) to find out whether you can proceed.  You must publish notice of the proposed name change in a newspaper for three weeks.  You may submit your request for a final order changing your child’s name 10 days after the publication is complete.

Petition for Change of Name for a Family – If you would like to legally change the names of the adults and children in a family, complete the following documents.  Please note that typically, BOTH PARENTS must consent to the children’s name change.  If you cannot obtain the other parent’s consent, contact the judicial law clerk (after you know which judge is assigned to your case) to find out whether you can proceed.  You must publish notice of the proposed name change in a newspaper for three weeks.  You may submit your request for a final order changing the names 10 days after the publication is complete.

MOTIONS TO WAIVE PUBLICATION AND SEAL FILE
To obtain a name change, you must publish the Notice of Petition for Change of Name in a newspaper for three weeks.  If you believe your personal safety would be at risk by publishing the Notice, you may complete the documents below to ask the judge to waive the publication requirement and seal your case.
